Module execution 

Ordered application execution and acknowledged response cache. Ordered RPC execution/cache adapter; see docs/rpc-execution-controller-contract.md.

TRUST: decided kind/session/key and application bytes come from immutable, authenticated admission plus ordered consensus. This module does not supply that boundary, checkpoint restore, or authentication. ACKs are separate decided control operations. Never use a transport acceptance as an ACK. One application operation is active; client sequence order may have holes. Reset/fence is terminal; presented tokens and uncertain effects cannot retry.
